This is a list obtained from within resource3.glg of each level folder. Some of them are missing on the map, but should someone figure a way to edit map, only map editing is needed (executions anims and everything else should be already packaged in other files in the level folder).
Things you are not getting during normal playthrough (without trainers or not included in the map) are marked yellow.
Awakening:

Nightstick is caried by asylum staff in cutscenes, but you don't get to execute any of cutscene staff without trainer.

Sexual deviants:

Red light:

Light handgun is caried by the hunters that start shooting at you at the end of the level. You cannot get their weapons without trainer, even if you manage to kill them (sometimes, they stay on the ground shooting somewhere else, when that happens, maybe you can kill them by shooting bricks at them, but usually they are placed on the same level above the earth as you, you can't aim a lure)
